- All procedures require consultation services. Each procedure requires a separate review. Consultation services are nonrefundable (no exceptions) and are paid in-full when scheduled. The selected procedure must be initiated and completed within 6 weeks of the associated consultation.

Services must be canceled within 48 hours. Failure to cancel within 48 hours of the appointment will be called a “no show” and will be subject to a $100 fee (ie, the $100 deposit will serve as the “no show” fee).

All services are to be scheduled with a $100 deposit. The $100 deposit will be applied to the cost of the selected procedure at the time of procedure
